Requirements

This program has been developed and offered by OceanMist Dental Office specifically for clients and patients who have no access to a dental issuance. It is designed to allow some measure of affordability and relief for cash patients who would like to become members for extended period of time. The “In House Membership” program is not designed to replace dental insurances nor should it be considered as one. However, members can enjoy a vast array of, mostly, non-cosmetic treatments offered by our dental practice at more economical fees. Most preventive, restorative, endodontic, fixed and removable prosthodontics procedures will be included with membership.

The Basics

Members may start enjoying their dental benefits immediately after the proof of initial payments (e.g. processing of checks) and all memberships expire exactly 1 year (365 days) later at closing time and/or 6:00 PM.

Members may enjoy their dental benefits during operating hours and based on appointment availability only.

Memberships do not extend to holidays, weekends and weekdays when OceanMist Dental is closed for various reasons.

All rules, regulations and policies with respect to operation of OceanMist Dental Office will be enforced, applied and followed. Such guidelines may include but not limited to demeanors, payment agreements, dental forms, late payment charges and etc.

The benefits are only applied to procedures performed at OceanMist Dental Office by general dentists. Specialist fees and referrals require full payments charged by other healthcare providers and are not covered under membership agreements.

Required medications and fees charged by pharmacies are not covered under the membership agreements.

Fees are calculated and patients are charged per procedures. Additional charges may apply for related, necessary and needed tasks.

In House Membership at OceanMist Dental is not valid with other offers, insurance memberships, proposals and propositions.

A basic charge of $ 10.00 per person per month applied on a monthly or annual basis in order to initiate the membership process.

This basic fee is “Non-Age Related” and will be applied to each individual, regardless.

You must be over the age of 18 in order to apply for membership.
Patients below the age of 18 may join alone with parents and with proof of parental guardians and relationships.
A minimum of 1 year membership is expected for all members. All annual membership fees are due and collected at the start. Fees for membership are non-refundable, non-transferable and non-negotiable.
OceanMist dental reserves the right to refuse and/or terminate memberships based on credit standings, conducts, expectations and incompatibility.
OceanMist dental reserves the right to evaluate, alter, omit, add and modify any amendment within the agreement without prior notice. The modifications may take effect immediately and all possible steps to advise members of such changes will be taken to ensure full disclosure of the adjustments.
